I've spent 20+ years building and running technology organizations in enterprise SaaS, the last 11 at VisiQuate, where I lead a 200+ person engineering organization as CTO. The last few years have been about one thing: taking a regulated healthcare SaaS company through a real AI transformation — not pilots and demos, but production systems inside HIPAA, SOC 2, and HITRUST constraints. What that looks like in practice: — A governed AI platform in production: one secure entry point for the whole team, hundreds of governed tools, PHI sanitization on every response, and model access only through BAA-covered providers. No public endpoints, no exceptions. — Production AI agents doing real work: AI review of 100% of merge requests, AI-driven database migrations with automated verification, and a multi-agent software-delivery pipeline with human approval gates. — Governance as a first-class deliverable: AI code-production policy — risk tiers, cumulative approval gates, and non-waivable hard stops for PHI exposure. In healthcare, the boundary is the product. — The results: ~30% productivity gains across engineering and services, ~60% of development work AI-assisted, and AI spend cut 50–65% through tiered model routing. Capacity grows without headcount growing. Alongside the AI work: ~50% infrastructure cost reduction while doubling production users, onboarding timelines cut up to 40%, and a platform re-architecture toward metadata-driven multi-tenancy. My operating belief: the durable advantage isn't the model — it's the structure around it. Governance, context, orchestration, and the discipline to ship at 80% and iterate.
